This report has four required sections. Please read the descriptions closely, complete the sections below, and submit.

In section four, please select all the enclosures you will attach, and then attach the files at the end.

We strongly suggest that online submissions first be saved in a Word document, then copied and pasted into the online form. There is no way to save the online form while you work on it. Responses could be lost if you step away, your internet browser times out or refreshes, etc.

Grant Activities
This section is the heart of your grant report. Please recap what your organization proposed to do with the grant and how the project was implemented. Highlight any areas in which project execution exceeded your expectations, as well as any areas of difficulty. Two or three paragraphs are sufficient.

Outcomes and Impact
Share data or stories that illustrate the difference this project has made to your Catholic entity and/or community. Perhaps revisit the needs and expected outcomes you included in your initial grant proposal and detail whether they were met.

You may or may not have a lot of numbers or quantifiable data. That’s okay! The Outcomes and Impact section is a great place to share participant quotes, testimonials, or stories of transformation—any good news that is a direct result of your project.

Testimonials, quotes, and stories are of critical importance to the Catholic Foundation of Eastern Montana and your grant impact report will be considered incomplete without them (which factors into future grant award decisions).
